Caja and private healthcare: plan for both questions
Enrollment, access, insurance and the Medicare issue American retirees should resolve before moving.

Start with coverage and access
Costa Rica’s public social-security institution is the Caja Costarricense de Seguro Social, usually called CCSS or the Caja. Ask CCSS and your immigration adviser what enrollment and contributions apply to your status. Request an assessment based on your actual case instead of relying on a universal percentage quoted in a forum.
Then make a practical care plan: which clinic you would attend, which specialists you need, how referrals work and how you will get there. An enrollment certificate is not a travel-time estimate or a promise of a particular appointment date.
Compare private options carefully
Ask an insurer to explain deductibles, pre-existing-condition terms, age limits, renewability and the provider network. Check whether it pays a provider directly or reimburses you after you pay. For regular treatment, obtain a quote from the provider and verify that the proposed plan covers it.
Some people combine public care with private appointments. Whether that makes sense for you depends on your health needs, access and budget. Do not cancel an existing policy until the replacement coverage and effective date are confirmed.
Medicare is not your everyday Costa Rica plan
Medicare’s overseas-coverage fact sheet says coverage outside the United States is limited and most foreign care is not paid by Medicare. Certain supplemental or other plans have additional provisions, but travel-emergency cover should not be assumed to cover permanent overseas living.
Before dropping any Medicare component, ask about future enrollment rights, possible penalties and coverage when visiting the U.S. That is a different decision from choosing local care.
Bring your care information with you
- Prepare a summary of diagnoses, allergies and current treatment.
- Record generic medicine names and confirm local availability with a clinician.
- Keep prescriptions and relevant records accessible.
- Identify the appropriate emergency facility for your location.
- Set aside funds for care that requires payment upfront.
Digital nomads should separately check the program’s insurance requirements; a general healthcare plan is not automatically proof of compliance.
